What causes varicose veins?
Varicose veins are caused by faulty valves inside the leg veins. Healthy venous valves open to allow blood to flow upwards toward the heart, then close to prevent it falling back. When these valves weaken or fail, blood pools in the vein under the effect of gravity, causing it to stretch, enlarge, and become visible beneath the skin as a twisted, bulging vein. The condition is strongly hereditary — if one or both parents had varicose veins, your risk is significantly increased. Other contributing factors include pregnancy, prolonged standing, increasing age, obesity, and previous deep vein thrombosis (DVT).

Are varicose veins dangerous if left untreated?
Varicose veins are rarely dangerous in the short term, but they are a progressive condition — they do not get better on their own and will gradually worsen over time. Left untreated, varicose veins can cause significant leg pain, heaviness, aching, itching, and swelling. In more advanced cases they lead to skin discolouration, lipodermatosclerosis (hardening of the skin and fat), venous eczema, and eventually venous leg ulcers — which are chronic, difficult to heal, and seriously affect quality of life. Bleeding from a varicose vein, though uncommon, can be alarming and occasionally significant. Early specialist assessment is always advisable.

Can varicose veins come back after treatment?
Recurrence is possible with any form of varicose vein treatment, including both surgical and endovenous approaches. The most common reason for recurrence is an underlying venous reflux pattern that was incompletely mapped or treated at the time of the original procedure. What is venous reflux disease?
Venous reflux disease occurs when the valves inside the leg veins fail to close properly, allowing blood to flow backwards (reflux) and pool in the lower leg rather than returning to the heart. It is the underlying cause of the vast majority of varicose veins and can produce significant symptoms — leg heaviness, aching, swelling, restless legs, and skin changes — even before visible varicose veins have developed on the surface. Can varicose veins cause leg swelling?
Yes. Chronic venous insufficiency — the advanced form of venous reflux disease — is a common cause of ankle and lower leg swelling, particularly at the end of the day or after prolonged standing. Raised venous pressure forces fluid from the blood vessels into the surrounding tissue, causing pitting oedema. This is often dismissed as a normal consequence of ageing, but it can frequently be traced to treatable venous disease. Treatment of the underlying venous reflux typically resolves the swelling progressively over the weeks and months following intervention.

What is EVLA and how does it work?
Endovenous Laser Ablation (EVLA) is the gold-standard, NICE-recommended treatment for varicose veins caused by great saphenous vein (GSV) or small saphenous vein (SSV) reflux. Under ultrasound guidance, a fine laser fibre is passed inside the affected vein through a small needle entry point. Tumescent local anaesthetic is then injected around the vein to numb the area and protect surrounding tissue. The laser is then activated as the fibre is slowly withdrawn, delivering controlled heat energy to the vein wall, causing it to seal permanently. Blood is immediately redirected into healthy deeper veins. The whole procedure takes approximately 60 to 90 minutes and patients walk out of the clinic the same day.

Is laser vein treatment painful?
EVLA is performed entirely under local (tumescent) anaesthetic, so the procedure itself is not painful. The injection of local anaesthetic involves multiple small needle stings along the length of the vein — this is the most uncomfortable part for most patients and is usually well tolerated. During the laser treatment itself, patients typically feel mild warmth or pressure but no significant pain. In the days following treatment, some aching, bruising, and tenderness along the treated vein is normal and settles quickly with simple analgesia such as ibuprofen. The vast majority of patients return to normal activity within 24 to 48 hours.

Can I walk home after EVLA treatment?
Yes — walking immediately after EVLA is not just permitted but actively encouraged. Gentle walking helps promote blood flow through the deep veins and reduces the risk of superficial thrombophlebitis. EVLA at Sheffield Vein Clinic is a true walk-in, walk-out procedure performed entirely under local anaesthetic. No sedation or general anaesthetic is required. You should arrange a lift home on the day of treatment as driving is not recommended immediately after the procedure. Most patients with desk-based jobs return to work the following day.

What is foam sclerotherapy?
Foam sclerotherapy involves injecting a medically prepared foam — made by mixing a sclerosant drug with gas — directly into a varicose vein under ultrasound guidance. The foam fills the vein and causes controlled inflammation of the vessel wall, leading the vein to close and be gradually absorbed by the body over several weeks. It is most commonly used to treat varicose tributary veins alongside EVLA, or as a standalone treatment for smaller varicose veins where EVLA is not required. No anaesthetic is needed and there is no meaningful recovery time — patients can resume normal activities immediately.

What is microsclerotherapy for thread veins?
Microsclerotherapy is the gold-standard treatment for thread veins (also called spider veins or telangiectasias) on the legs. Using an ultra-fine needle, a very small volume of sclerosant solution is injected directly into each thread vein under bright lighting and magnification. The solution damages the inner wall of the vessel, causing it to collapse and fade over the following four to eight weeks. Several treatment sessions spaced six weeks apart are typically required to achieve the best cosmetic result, depending on the density and distribution of thread veins present.

What is Thermavein treatment?
Thermavein uses a very fine needle-like probe to deliver a precisely controlled microwave thermocoagulation current directly into thread veins, causing them to close on contact. Unlike sclerotherapy, no injection of chemical is required — the vein is destroyed by heat alone. Thermavein is particularly effective for facial thread veins, thread veins around the nose and cheeks, and small leg thread veins that have not responded well to sclerotherapy. Treatment sessions are short, results are immediate, and downtime is minimal.
